B vitamins are usually sold as a complex, which is right, because they operate as one system. Methylation — the reaction that builds neurotransmitters, repairs DNA and regulates gene expression — needs B12, folate and B6 together, and runs poorly if any one is short.
The form problem
- Folic acid is synthetic and must be converted before use. A substantial share of people carry MTHFR variants that convert it slowly, and unconverted folic acid accumulates
- Methylfolate is the active form and bypasses that step entirely
- Cyanocobalamin is the cheap B12 and must also be converted; methylcobalamin and adenosylcobalamin are the forms the body uses
- B6 as pyridoxine converts to pyridoxal-5-phosphate, which some people do poorly — and high-dose pyridoxine over long periods can cause peripheral nerve damage, which the active form does not
Who is actually short
B12 deficiency is common and under-diagnosed. Vegans and vegetarians are the obvious group, but so is anyone on long-term metformin or acid-suppressing medication, both of which impair absorption, and older adults, in whom stomach acid declines.
The standard serum B12 test is a poor instrument — it measures total B12 including the inactive fraction. Methylmalonic acid or homocysteine reflect functional status far better and are worth asking for when symptoms and a normal B12 disagree.
B12 deficiency causes neurological damage that becomes permanent if it goes long enough uncorrected. It is one of the few nutrient shortfalls where being slow to act has a lasting cost.
Practical guidance
- Prefer methylated forms; they cost slightly more and work for everyone, including the people who do not convert well
- A minority feel wired or irritable on methylfolate. If that happens, a lower dose or the hydroxocobalamin form usually resolves it
- Homocysteine is the single most useful marker of whether the system is working — it rises when B12, folate or B6 are functionally short
- Do not take high-dose B6 as pyridoxine long-term. This is a real and well-documented harm
- Food first: liver, eggs, fish and leafy greens cover the group between them
